MRI is a extremly useful tool in pre and postoperative staging of breast cancer and screening for patients at increased risk (familial breast cancer, BRCA 1/2 carriers). However, it not a stand alone screening modality as for instance DCIS 25-40% does not show an enhancement and is only detectable by microcalcifications.
MG (for micrcalcs- resolution of MRI is too low) and US is mandatory in breast cancer screening, but in as said above MR-MG is a most valuable adjunct especially for patients at increased risk.
In patients with breast cancer THE most best evaluation of disease (multifocality, multcentricity) as of the kontralateral breast is MR-MG.
In conclusion, MR-MG definitely changes decision making in clinical routine, management of high-risk patienst and pre- and postoperative staging.
