Dear Dr Wael, There is total gastrectomy with esophago-jejunal anastomosis. This is the usual operation surgeons do considering prepyloric vein on the outer surface of the stomach to do total gastrectomy, Sometimes the surgeon didn't notice the prepyloric vein and consequently a bit of gastric antrum is remained with the duodenum. As the gastric antrum is the main provocative gastrin secretion, patient could attain ulcers post-operatively. Here you can see the remaining of the gastric antrum with duodenum.
