ACCREDITATION
This CME activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the Joint Sponsorship of the Institute for Medical Studies (IMS) and First Hill Diagnostic
Imaging. IMS is accredited by the ACCME to provide continuing medical education for physicians.

CREDIT HOURS
IMS designates this educational activity for a maximum of 33.5 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

CANCELLATIONS
If you must cancel, please call (206) 329-0304 one month prior to the fellowship date. An administrative
fee of $500 will be assessed after that date.
