Acute Cerebral Infarct

Acute MCA territory infarct
This 82 year old lady with a history of atrial fibrillation presented with acute left hemiplegia. On the non contrast CT scan of the head there is low density change and loss of the grey and white matter differentiation at the right insular cortex (cotical or insular ribbon sign). There is also partial low density change in the right lentiform nucleus in keeping with an acute middle cerebral artery (MCA) territory infarct. In addition there is a hyperdense right MCA (click image) in keeping with thrombus in the vessel. Progress CT scans showed further evolution of the infarct.
