Perpendicular splenomegaly is usually due to a sympathetiic effect of the spleen in the inflammatory disorders but inward oriented splenomegaly, like this case, takes part of some systemic lymphatic disorder like leukemia and so on.

So obvious but what's the reason for this huge splenomegaly in this case? Any clue in this film?

Norrow slit standing between liver and huge splenomegaly would be nothing but only the stomach. Stomach is the only air containing organ at the upper abdomen under diaphragm, and i think you can delineate it even without barium once you see the body and gastric antrum with its familiar shape. Thanx for comment.

splenomegaly is clear, although in such a case I'll comment with huge soft tissue density for ultrasound or CT correlation. the pressure effect on stomach needs barium study.
