Well guys, I've to apologize...the case be misleading since the contrast enhanced image may mimic meningitis.
By the way there are some findings that are not consistent: the first is that the "meningeal" enhancement is serpiginous along the posterior aspcet of the medulla (not typical at all !) and the second can be identified on the T2-w image (can't you see those tiny hypointense spots behind the medulla ?). Any guess ?

arachnoiditis ? and is the tip of dens replaced by granulation tissue. ? only two images seems insufficient..

There's C6-7 herniation, yes. By the way that is not the most important finding !!!!!!
Tip: is contrast agent administrati usually performed to evaluate disc herniation ? if not, which is the relevant finding in the contrast enhanced sequence ?

Disc Herniation at C6-7, with cord impingement. This image is very similar to what my Cervical MRI looks like...:(
