This is a differential diagnosis with Hirshsprung or imperforated anus apparently but a lateral radiography will show impacted meconium in the rectum of the neonate with bowel obstruction.

Thanks for your unique cases.
Of the Meconium plug syndromes is small left colon syndrome which is functional immaturity of colon; a large segment of large bowel usually the descending colon is small with distended rectum upon barium enema. In Hirshsprung the narrowed large bowel segment is small with reversed recto-sigmoid ratio...i.e. proximal colon is larger than rectum in diameter.
