Thank you for your comments seed rad on my two cases.
In addition to the pneumo-peritoneum, there is a peritoneal fluid level on the left side, so it is not a simple air under diaphragm, it is rather peritonitis after some perforation of the some viscus or simply hydro-pneumo- peritoneum.
