Subphrenic abscesses are usually apparent within 5 to 10 days after surgery.
Signs include fever,pain,pleural effusion prolonged atelectasis, pneumonia, and prolonged leukocytosis.
US and CT scanning are useful for identifying abscesses.

Looks like normal stomach bubble; CT is indicated.

It seems to me a pleural effusion and prolonged atelectasis. Please way to see 2 videos related with these plain films. To me it is remarkable the abdominal signs: The mass efect and the air-fluid level....did you get it?
