Unfortunately no CT. Thanks for all your answers! The answer is... drum roll.... foreign body aspirated within the left main bronchus resulting in airway obstruction demonstrated by persistent hyperinflation on expiratory chest radiographs. Good call Dr. Ref! The foreign body was a small lego-block which was very difficult to remove per the operative note.
air entrapment at the left lung with shifting of the mediastinum to the right in the expiratory film..I guess there's something obstructing the left main bronchus as there's some sort of proximal dilatation at the left hilum..
