* Failure of the lower esophageal sphincter (LES) to relax.
* Primary achalasia: associated with loss of ganglion cells in the esophageal myenteric plexus.
* Secondary achalasia is relatively uncommon. Etiologies include certain malignancies, diabetes mellitus, and Chagas disease.
