Is the plain film of abdomen efficient to diagnose acute cases in a cost-effective way?

In the setting of abdominal blunt trauma rupture of the second portion of duodenum, the most fixed part of the small intestin, may show air bubbles around right kidney and this can be clearly seen on the plain fims as shown here to start the discussion. Please give your opinion about the utility of the plain abdominal radiography as the first modality to diagnose acute abdominal cases.

hello Dr saeed,
i feel good plain radiographs can serve as a screening tool only. when i used to work in A&E setup few years ago, once we feel the patient is clinically having a problem we found a cect to be very conclusive in decision making .

Thank you for your reply to discussion. You are right on your idea about the plain films and if it is the first step for screening it should be of good quality. But if the modern imaging tools such as CT or US are not available plain film is the minimum requirement to find the way for complementary studies. Once i was teaching surgery residents, about 30 years ago, how to interpret plain films, and they were very enthusiastic to refer patients for plain radiographies and we found it very useful as the preliminary work which was most of the time conclusive. I agree with you that it seems a bit out-dated but still the resident of radiology in abdominal radiology should know interpreting plain radiographies. Modernized text-books have still kept their chapter for plain radiography. Anyhow there is no objection to go up to the end in acute settings but we can't neglect the role of the plain radiography altogether. Yours, s.rad
I FEEL PLAIN FILMS WHICH ARE PROPERLY TAKEN GIVE US THE DIAGNOSIS ALONG WITH HISTORY AND OTHER PARAMETERS INSPITE OF MODERN IMAGING.

True! Conventional radiology is not something to forget about, it still exists as a basic for medernized techniques. The whole area can be seen on a plain film of abdomen for example, but others show only a section and to view all we have to go through all sections and the cost is something to include in the price for final diagnosis..
