Portal venous gas on CT is associated with a wide range of pathologies that do not necessarily warrant surgical management.

Portal venous gas is usually peripheral and large while biliary gas is central and small.

Possibilities at our patient may include either bowel obstruction or gangrened segment of bowel from hernia.

Incidental finding is calcified hepatic focal lesion. Biliary fistula is most commonly caused by hydatid cyst.
